Below you will find some key documents that will help get you set up to join our schools as a member of the Dedham team.

Fingerprinting Information & Application
ALL public school employees undergo a fingerprint-based national Criminal History Record Investigation (CHRI) check. This one-time process involves submitting your fingerprints at a specially designated center. The fingerprints will be sent to the FBI and a report will be sent to the Criminal Justice Information Service. The final report will be sent to the Superintendent for review.
The fee is $55 for staff who hold a DESE license and $35 for those who do not hold a DESE license. You will need to pay with a credit card while online or indicate that you will bring a personal check with you to the appointment.

2024 CORI Background Check Authorization
- This for is required for all Dedham employees and volunteers, authorizing the District to run a criminal history check.
- CORI applications must be completed and returned to Dedham Central Office in-person with a valid, government issued photo id.

2024 W-4 Federal Tax Withholding Form
- This form sets your Federal Tax Withholding selections.
- It must be completed and submitted at the time of hire.
- Current employees can update this form only when legally applicable "life events" occur.

Form I-9, Employment Eligibility Verification
- Under federal law, employers must complete Form I-9, Employment Eligibility Verification, within 3 business days of when you begin to work for pay.
- They must also examine any documents that you provide to show your identity and employment authorization.

2024 Direct Deposit Enrollment Form
Direct Deposit is a safe and convenient way to have your earnings automatically deposited into your checking or savings account. This authorizes the Town of Dedham to deposit your paycheck directly into your bank account.
The form must be completed and returned in- person, with a copy of a voided check or bank authorization form showing account and routing numbers, to the DPS Payroll Office, 100 Whiting Avenue, Dedham, MA

M-4 State Tax Withholding Form
Massachusetts Employee's Withholding Allowance Certificate. The M-4 form tells the Town of Dedham how much to withhold for state income taxes.

Statement Concerning Your Employment in a Job Not Covered by Social Security
All new employees must fill out this form and it should be filed with their employee record as it acknowledges that they have been made aware that they do not pay into social security as members of the Massachusetts State Employees' Retirement System.

District Confidentiality Agreement
It is the policy of Dedham Public Schools to provide our employees or students with a level of privacy and confidentiality with any information concerning any of our employees or students.
All new employees are asked to sign a Confidentiality Agreement indicating that they understand the policies and legal protections required.
