Steps for Requesting DHS Transcripts
Graduated/Former students:
Requests for high school transcripts must be made in writing and mailed to:
Dedham High School Guidance Department

Requests must include your name at the time of graduation, year of graduation and date of birth. The cost of each transcript is $2.00. Please make checks payable to " Dedham High School ."
Current students:
You may request an official transcript (signed, stamped and sealed in an envelope) or an unofficial transcript by seeing your guidance counselor or Mrs. Goyette. Transcript requests are not always able to be filled immediately, so please make your request at least 48 hours before you need a transcript.
Seniors requesting transcripts for college applications:
Please print and submit the Transcript Request Form to your guidance counselor. Follow all instructions on the form carefully.
Seniors requesting mid-year reports for college applications:
Students must submit another business envelope with one stamp, addressed to the college admission office. Do NOT put your return address on the envelope. Once second-term grades are available, your counselor will send the mid-year report.
Graduating seniors who need transcripts for colleges they will attend:
Please inform Mrs. Goyette of the college you plan to attend as soon as you make your decision (usually by May 1st ), and no later than May 31st for those students waiting to hear back from wait-lists. Your final transcript will be automatically sent to the college of your choice by the Guidance Department. No other action by you is necessary.
