GENERAL INFORMATION AND INSTRUCTIONS
Information:
· Community Service forms are available in the main office and in the guidance office.
· Community Service Directories are located in the following locations: Main Office, Offices of the Assistant Principals, Guidance Department, Library, and the Dedham Youth Commission.
· Court- mandated community service and other community service obligations DO NOT count towards graduation requirement community service hours.
Instructions for the Community Service Form:
· Research community service options in the "Community Service Directory" or on your own.
· Decide on a community service agency.
· Pick up the DHS Community Service Form in the office of Mr. Forrest(grade 10,11) or Mr. Murray (grades 9, 12).
· Complete the Community Service Form, with the exception of "total hours completed with supervisors initials".
· Get approval from either Mr. Murray (grades 9, and 12) or Mr. Forrest (grades 10 and 11) prior to completing any community service hours.
· Once hours are completed, have the agency supervisor's fill out the selection labeled "total hours completed with supervisor's initials".
· Photocopy your own completed Community Service Form. Keep a copy for your own records.
· Drop completed form off to Mrs. Goyette in the Guidance Department.

Recommendations for Volunteer Sites
· Sign In Logs are recommended to serve as a record-keeping tool for volunteers and their supervisors.
· Supervisors should schedule a meeting with their student volunteer prior to the start of community service to outline agency/site expectations including punctuality and issues related to confidentiality.
· The volume of student volunteers is at the discretion of the agency.

DHS Student Volunteer Expectations
· Students are expected to arrive on time to the supervising agency.
· Students must contact their site supervisor AS SOON AS POSSIBLE if they are unable to get to the site on a previously agreed upon day.
· Students are expected to act responsibly.
· Students should use appropriate language and be appropriately dressed.
· Students must maintain a contact log of dates attended and hours completed at the site.
· Students must meet with their site supervisor prior to the start of community service to outline site expectations.
· CLASS OF 2007: Hours must be completed with necessary forms submitted on or before May 1, 2007.
